JOB STATEMENT/SUMMARY
The behavioral health therapist operates as part of the primary care team for patients at Coplin Health Systems, offering recommendations regarding behavioral health interventions to primary care providers, conducting brief interventions requested by primary care providers, and providing behavioral health services.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES OF THE POSITION
- Provides behavioral health services to patients, including scheduled and same day consultations.
- Maintains a visible presence to primary care providers during clinic operating hours and is available for brief consultative interactions with primary care providers throughout the day.
- Provides primary care providers with same-day feedback on client encounters.
- Performs brief, limited follow-up visits for selected patients.
- Provides a range of services including screening for common conditions, risk assessments, and interventions related to chronic disease management programs.
- Provides psycho-education for patients during individual and group visits.
- Assists in the development of clinical pathway programs, group medical appointments, classes, and behavior focused practice protocols.
- Maintains an up-to-date library of patient education materials for commonly seen problems.
- Identifies, reviews, and modifies educational materials for literacy level and cultural appropriateness.
- Triages patients with severe or high-risk behavioral problems to community resources for specialty services consistent with Coplin policies and procedures.
- Presents the primary care-behavioral health model through networking and outreach to private and public programs and agencies, in order to establish effective linkages and resources.
- Maintains clinical records and other necessary paperwork as required by Coplin's policies and procedures.
- Educates primary care providers in the basic principles of brief behavioral and cognitive behavioral interventions and reinforces their use in the medical visit.
- Supports collaboration of primary care providers and psychiatrists concerning medication protocols while staying within their scope of practice.
- Provides assistance in capturing program evaluation and fidelity measures.
- Attends clinic meetings, including all staff, primary care provider, clinic leadership, and clinic primary care-behavioral health committee meetings as requested.
- Performs other duties as assigned.

Education (level and type) and Related Work Experience
A master's degree in a social science field is required. A current and valid state license to practice behavioral health in the state of West Virginia, and good standing with all appropriate licensing agencies is preferred. The candidate must be working towards licensure for consideration for this position.
