As a Case Manager with REM/The MENTOR Network you’ll be one of those special people.  For REM and The MENTOR Network, a Case Manager is critical to our success and exemplifies the wonderful mission driven work we do here every day.  To that end, the Case Manager role is a fantastic role to continue your career path in Social and Human Services with a myriad of career options to move up and across the organization.  REM WV/The MENTOR Network is seeking a Case Manager to meet and work with individuals to gather information and assess each client/consumer’s needs to include the assessment of the cost of care.  The Case Manager may prepare a plan of care, and identify and implement services to meet an individual’s needs.

Requirements:

  • Bachelors degree in Human Services or equivalent in education and experience required
  • In addition, one year of related work experience and knowledge of case management
  • Experience working with Title XIX Waiver program rules, regulations and limitations
  • Other education and experience plus licensure as required by state
